Nourish & Heal LA is a community-driven initiative with a mission to bring nutritious meals and essential healthcare assistance directly to the aging population of Los Angeles—completely free of charge. This proposal seeks funding to support the development, implementation, and expansion of this program across vulnerable neighborhoods in Los Angeles County. By addressing two key social determinants of health—nutrition and access to care—we aim to uplift the health, dignity, and independence of seniors in our community.
